MEMBERSHIP APPLICATION
All applications must include NAME and CERTIFICATE # of TWO (2) SPONSORS.
Proof of residence for Members and Associate Members is to accompany applications. Photocopies of valid N.Y.S. Driver's License or Non-Driver's Identification are accepted. Other documents are acceptable on a case by case basis.
A NON-REFUNDABLE application fee of $40.00 must accompany all applications. Membership may be denied if applications are incomplete, if inaccurate information is provided within the application, or if negative information disclosed through criminal or financial background investigation.
